Have you been a victim of fraud?

If you believe that you have been, or are likely to be, a victim of fraud (including identity fraud) you can request to ‘freeze’ your credit report.

This will mean that during the ‘freeze’, Centrix will “suppress” the credit information held on your credit file and will not, except in limited circumstances, provide your credit report to credit providers or update or add information to your credit file.

If you feel you have been a victim of fraud and would like to apply to have your credit file suppressed, please fill in the form below.

Find more details on suppressing your credit file here.

We have an agreement to forward ‘freeze’ requests with the two other credit reporters (Equifax and illion) and can pass these on so you will not need to contact them separately if you agree to this.

If you have concerns about scams, or think you have been a victim of fraud or identity theft, you can contact IDCARE free of charge to get specialist help and practical advice. Visit their website here.

    Privacy Notice

    Use of your information

The information you provide us is used to confirm your identity and to satisfy ourselves that we are supressing (putting a “freeze” on) your Centrix credit file and not someone else. If you do not provide this information, we may not be able to freeze your credit file. If we cannot satisfy ourselves of your identity, we may require further information from you, or we may not be able to freeze your credit file.

Update or create your Centrix credit file

Unless you tick the box below to opt out, your name, address and date of birth that you provide us will update your Centrix credit file to ensure we have the most recent and accurate information, or if you we do not currently have a credit file, will be used to create a record. This information may be available to our subscribers when checking your Centrix credit file in the future. For further details see our Privacy Statement.

Forward your “Freeze” request to other credit reporters

If you consent, we can forward your request to the two other credit reporters in New Zealand (Equifax and illion) that may have a credit file on you. This means you will not need to contact these credit reporters to make an initial freeze request. This will require us to pass on to Equifax and illion the information you have provided to us on this form.

If you do not hear from Equifax or illion about your request, or have any queries about the request we have passed on, please contact them direct.

Access, Correction and Summary of Rights
